  • 1Dense white opacification mapping the right upper lobe.
  • 2The horiztonal fissure is retracted aggressively upwards laterally.
  • 3Medially, the hilar border is convex downwards, generating the classic reverse 'S' shape.
  • 4CT visualizes an aggressive massive central tumour crushing the upper lobe bronchus alongside para-aortic lymphadenopathy.
